One of the most common mistakes private practice owners make is evaluating equipment purchases solely on the monthly loan payment rather than the economic return the equipment can generate. Whether purchasing an OCT, retinal imaging system, visual field analyzer, dry eye treatment platform, or specialty contact lens technology, the critical question should never be:

"Can I afford the payment?"

Instead, the question should be:

"Will the equipment generate sufficient revenue and profit to justify the investment?"

Successful practice owners think like investors. Every dollar invested in equipment should be expected to generate a measurable financial return while simultaneously improving patient care, practice efficiency, and long-term practice value.

This article illustrates how to calculate return on investment (ROI) using the example of a $75,000 retinal imaging system financed for 60 months at 6.00% interest.

Step 1: Calculate the True Cost of the Investment

Many doctors incorrectly assume that a $75,000 equipment purchase costs $75,000.

In reality, financing costs increase the total investment.

Equipment Financing Assumptions:

  • Equipment Cost: $75,000
  • Interest Rate: 6.00%
  • Loan Term: 60 Months
  • Monthly Payment: Approximately $1,450

Total Payments: $1,450 × 60 months = $87,000

Total Interest Paid: $87,000 - $75,000 = $12,000

The true investment over the financing period is approximately: $87,000

This figure should be used when evaluating long-term return on investment.

Step 2: Determine Revenue Opportunities

The next question is:

How will the retinal imaging system generate revenue?

There are generally four sources of economic return:

Direct Testing Revenue:

Many practices bill retinal imaging as a medically necessary diagnostic procedure when supported by documentation and payer guidelines.

Assume:

  • Average reimbursement per test: $40
  • Number of tests per month: 100

Monthly Revenue: 100 × $40 = $4,000

Annual Revenue: $4,000 × 12 = $48,000

Increased Medical Eye Care Revenue

Retinal imaging often identifies pathology requiring:

  • Follow-up examinations
  • Additional diagnostic testing
  • Medical management visits
  • Referrals and co-management

Assume:

  • 10 additional medical visits per month
  • Average reimbursement per visit: $125

Monthly Revenue: 10 × $125 = $1,250

Annual Revenue: $15,000

Advanced retinal imaging can improve:

  • Patient confidence
  • Recall compliance

Assume the practice retains:

  • 20 additional patients annually
  • Average revenue per patient: $400

Additional Annual Revenue:  20 × $400 = $8,000

Step 3: Calculate Total Annual Revenue Impact

  • Direct Retinal Imaging Revenue: $48,000

  • Additional Optical Revenue: $10,000
  • Total Incremental Revenue: $81,000 per year

Step 4: Calculate Incremental Profit

Revenue is not profit.

Some expenses must be deducted.

Assume:

Variable Costs - Software fees, maintenance, image storage, staff time:  $6,000 annually

Incremental Operating Profit:  $81,000 - $6,000 = $75,000 annually

Step 5: Calculate Annual Debt Service

  • Annual Equipment Payment:  $1,450 × 12 = $17,400 annually

Step 6: Calculate Net Cash Flow Benefit

  • Annual Incremental Profit: $75,000
  • Less Annual Loan Payments: ($17,400)
  • Net Annual Cash Flow: $57,600

This means the equipment generates approximately: $57,600 of additional annual cash flow after paying the lender.

Step 7: Calculate Payback Period

The Payback Period measures how quickly the investment recovers its cost.

  • Investment: $75,000
  • Annual Cash Flow Benefit: $57,600
  • Payback Period: $75,000 ÷ $57,600 = 1.30 years or approximately 16 months

This means the practice recovers the full equipment investment in roughly sixteen months.

Step 8: Calculate Return on Investment

Traditional ROI Formula:  ROI = Net Profit ÷ Investment Cost

  • Annual Profit: $75,000
  • Investment: $75,000
  • ROI: $75,000 ÷ $75,000 = 100% Annual ROI

In other words, the equipment generates profit equal to its purchase price every year.

Step 9: Evaluate Return Over the Full Loan Term

  • Five-Year Revenue: $81,000 × 5 = $405,000
  • Five-Year Profit: $75,000 × 5 = $375,000
  • Total Equipment Cost with Interest: $87,000
  • Net Economic Benefit: $375,000 - $87,000 = $288,000
  • Five-Year ROI: $288,000 ÷ $87,000 = 331%

This means the practice earns more than three times its investment over the financing term.

Many ROI calculations underestimate the value of technology because they only consider direct reimbursement.
