Corporation

• EIN Application (Employer Identification Number)
• Optometric License of owner(s)
• Articles of Incorporation - Secretary of State filing page
• Fictitious Name Statement - If the corporation is doing business under a name other than the corporate name

Partnership/Limited Partnership

• EIN Application (Employer Identification Number)
• Optometric License of each partner
• Partnership Agreement
• Fictitious Name Statement - If the partnership is doing business under a name other than the partnership name

Society/Association

• TIN (Tax Identification Number)
• Association Agreement displaying title of organization
